GENERAL TERMs OF TELECOMMUNICATION EQUIPMENT TESTING (SK NO. 1A/KABALAI/2005)"Test House"

Article 2 : Obligations of Client and Rights of Examination Center of Telecommunication Equipment

2.1 For the Customer Premises Equipment (CPE), sample to be tested are 2 units, while for the
things non Customer Premises Equipment (Non CPE) is 1 unit.
2.2 If needed and in certain cases, Test House is entitied for talking back the sample in test and
will reexamine the test onto those sample.
2.3 If the client is still indebted or fail to transfer the payment, Test House is entitled to cancel
and not doing the testing to the equipment.

Article 3 : Payment and Expenses

3.1 Payment is transferred to Bank Mandiri, by account number 1030061999997, on behalf of
Ditjen Postel.
3.2 Amount of testing fee as according to Government Regulation No. 28 year 2005 about state
Acceptance of Non Iease which is used at Department of Communication and Information
Technology of The Republic of Indonesia.

Article 4 : Delivery of Sample Examination

4.1 Sample is delivered to Test House accompanied by proof of payment transfer.
4.2 Implementation of testing by Test House is done during 45 workdays by queue system.

Article 5 : Discharge of Responsibility

5.1 Test House is not responsible for all broken sanple which is tested, and client hereby walves
the right, to claim the Test House, officials or employees thereof on the loss and all
consequences arising out of or in connection with equipment testing.
5.2 Sample left more than 60 days will not be the responsibility of Test House.

Article 6 : Recapitulation of The Test Results

6.1 Recapitulation of the test results will be sent by Test House to Directorate of
Standardization.
6.2 Client will be able to obtain certificate at the Directorate General of Post and
Telecommunications through the Directorate of Standardizaton.

Article 7 : Amendements and Deviation of General Terms

7.1 Test House is entlited to rectify/amend/complete the general Terms of at any time without
prior Notice to the client. Notification will be notified to the client in writing or withan
announcement at Test House.
7.2 Client shall be obliged to comply with amendement, rectification or addition of the General
Terms.
7.3 Any deviation from the General Terms shall only be effective if it has been approved in
writing by Test House.
